Certified Employee Sick Leave Bank is open to enrollment during the months of August through October each year. Interested employees must submit the Authorization/Donation form to HR to become a member.
Employees desiring to take leave should complete the attached Employee Leave Form identifying the appropriate type of leave to be taken.

We will furnish the requested form within 30 days from our receipt of your request.  If you request a copy of your form and have opted-in to electronic delivery, it will be delivered via MyBenefitsChannel.  If you request a copy and have NOT opted-in to electronic delivery, a paper copy will be mailed to your address we have on file.
