Lauderdale County Health Department and Lauderdale County School System have partnered to provide seasonal influenza (flu) vaccinations at your child’s school. If your child is uninsured, there is no cost to you for the flu vaccine. If your child is covered by insurance including TennCare and private insurance, the Health Department will file a claim with the insurance plan and receive reimbursement directly from the insurance plan. There is no out of pocket cost to you for the flu vaccines provided during this school flu vaccination clinic. Your child will be bringing home a consent form beginning of this week, if you want your child to receive the flu vaccine please complete both sides. Be sure to sign the form, this will be your consent for the student to receive the vaccination. If you have any questions you can contact Michelle Brazier, Director of Coordinated School Health at 731-221-0959.
